HOTS Registration Packet

If you would like to train a teacher(s) for the HOTS Program, please download, print and complete the PDF registration packet found below and fax it back to the HOTS office at
(520) 795-8837.  One packet per school needs to be completed whether you are a new HOTS School or an existing HOTS school.
